Probably CCIE R&S; Voice needs real gear wilth extra modules like FXO/FSO; you need IP phones, VMware server with CUCM and licenses;
For R&S lab, u can get by with doing routing on GNS3.
Plus you need to have the underlying knowledge, best option is to gain the foundation by R&S and move to other areas. This is my personal opinion and what I am doing at the moment too...
Its always R & S because in R & S only you can learn the technology in depth and after all Voice run on IP only so you should know R & S and once you cleared R & S you can jump on to Voice. One more thing is R & S lab easily avaible but for Voice its not.

I think the best way to know the cost is i think is in this order for the CCIE exam from cheapest to not cheap
1) R&S, 2) security, 3) wireless 4) service provider 5)storage 6) Voice
I think it is like this due to the cost of the equipment, i just am not sure how much it cost for the service provider to know the equipment and so on.

You do not need CCIE R&S first in order to go for CCIE Voice; of course, it will not hurt, but it is not necessary.
The Recommended Path is CCNA >>CCNP Voice>>CCIE Voice
You may have to loook up few things from CCNP R&S if and when necessary.
If your interests are in Voice, then go for Voice. Forget about CCIE R&S.
